Intake forms
Build fully customizable, practitioner-branded intake forms with your own questions and consent documents, then send each client a private link. Responses flow straight into the client’s profile.
How do I create an intake form?
Go to Settings → Intake Forms → Forms and click Create New Form. In the builder you can set:
- Form Name and an optional description.
- Attached Documents — consent documents the client must review and agree to, each with a Required toggle and drag-to-reorder.
- Personal Information — turn on a demographics section and choose which fields to collect (name, birth date, sex, phone, email, address).
- Questions — add your own questions with a label, an answer format, and a Required toggle.
Click Save Form, or Preview Form to see the exact client-facing experience.
What question types can I use?
Each question can use one of these answer formats:
- Text and Text Area
- Select (Single) and Select (Multiple) — you provide the options, one per line
- Yes / No and Checkbox
- Date, Phone, and Email
How do I add consent documents?
Under Settings → Intake Forms → Documents, click Create New Document and write the title and content (for example, an informed consent or privacy policy). Documents are automatically versioned when you edit them, so past agreements always reflect the version the client actually signed. Attach any document to a form from the builder’s Attached Documents section.
How do I send a form to a client?
- Open the client’s profile and go to the Intake section.
- Click Send Intake.
- Pick a form under Select Template and click Generate Link.
- Copy the generated link and share it with your client.
How do clients fill it out, and where do responses go?
Clients open a simple step-by-step form — provider intro, any required documents to agree to, the personal-information section, your questions, and a final review before submitting. On submission, personal information updates the client’s record, and you get the full responses in the client’s Intake section (via View Responses) and in the central Responses tab.
Can I use one reusable link for new clients?
Yes. From the Forms list, use a form’s Copy link action to get a reusable intake link. Unlike a per-client link, submitting a reusable link creates a brand-new client in your practice — handy for a website “new client” link.
